We can be consumed by mental or physical pain. That does not mean we are selfish or self centered. The word "selfish" is often used loosely. Usually it doesn't even occur to a selfish person that they are selfish.
A person undergoing brain surgery is not selfish or self centered. Their brain needs attending. We have a self that has to be taken care of.
